Membership Forms

Upon becoming a member of Phi Alpha Honor Society - Beta Chapter at Michigan State University you will be asked to indicate which items you would like to purchase on your registration form. You will simply need to check the items that you would like and send a check, along with your registration form, to the address listed at the bottom of the form. The registration form includes the list of items and their prices.

To order additional merchandise, such as t-shirts, please visit the Phi Alpha Honor Society's national web site and click on the tab labeled "Merchandise" or use the link below.
